Instructions, Application, and Summary
Once you have completed a year of volunteering, and you are ready to apply for the PVSA, fill out the following documents and turn them into the office along with all of your Volunteer Tracking sheets to the office by April 30th of the current school year. Late submissions will not be accepted.
